  1. Facilitation of AMA/HCFA-Mandated Physician Charting Both the AMA and HCFA have become very specific regarding the charting requirements needed to defend billing for the various levels of service claimed. These requirements are complex and easily ignored by physicians. In order to assure physician compliance with the mandated charting requirements, the development of a totally new form of charting is needed. EDITS Chart uses a template-based format that guides physicians to complete the mandated elements. Other models of template-based charting are well established in the emergency medicine community. The unique aspect of EDITS Chart will be its incorporation of "check-off" charting in association with dictation or writing and the use of scanning technology to assist coders.

Counting of Completed Data Elements to Facilitate CPT and ICD-9 Coding and Room Charge Assignments The AMA and HCFA require that a specific number of elements be present in a history and physical examination to justify a selected level of service claim. In the performance of CPT coding, coders are required to count the elements of the history and physical examination to determine if enough of the required entries are present to support the desired service code. Optical mark recognition technology will allow entries to be counted and placed in decision algorithms in order to facilitate the work of coders. The coder would have the final say as to the determination of the appropriate level of service. This process would substantially facilitate coder work and improve productivity.
